OurHealthGrid is a personal health record and information management tool. It is designed to help you organise, track, and store your own health information in one place.
OurHealthGrid is not a medical service, clinic, or healthcare provider. It is not a substitute for professional medical advice, diagnosis, or treatment. It is not connected to or endorsed by the NHS or any healthcare body.
The Health Assistant uses AI to help you identify who to contact based on symptoms you describe. It is a signposting tool only. It does not diagnose conditions, provide medical advice, or replace the assessment of a qualified medical professional.
It only suggests one of four actions: call 999/A&E, call 111, see your GP, or self-care at home. Every response ends with a reminder to call 111 or 999 if unsure. The assistant always errs on the side of caution.
Do not delay emergency care based on anything the Health Assistant says.
To the fullest extent permitted by law, OurHealthGrid and its operators accept no liability for any loss, injury, illness, or death arising from reliance on information or outputs from OurHealthGrid, including the Health Assistant. Use of OurHealthGrid is entirely at your own risk.
OurHealthGrid does not connect to your NHS patient record or any official health record system. All information is entered by you manually. Nothing stored here updates your NHS record or is visible to any healthcare provider unless you share it directly.
When you upload documents and use AI extraction, the content is processed by AI to extract key information. Always verify extracted information against the original document. AI extraction is an organisational tool only — not a clinical interpretation.
Health information is special category personal data under UK GDPR. All health data is encrypted at rest. We never share your health data with other platforms without explicit opt-in consent. You can export or delete your data at any time.
